چکیده

both very common and widely prescribed agents for bowel preparation and diabetes control, respectively. Renal damage after usage of OSP and lactic acidosis after metformin are both well recognized adverse effects. Hence, there are regulations for each of these two agents to limit their use in specific populations. However, the safety of OSP usage in patients taking metformin is seldom discussed. We report here a diabetic patient on metformin who developed lactic acidosis and acute renal failure after ingestion of OSP. Case Report

Electrolyte Imbalances and Nephrocalcinosis in Acute Phosphate Poisoning on Chronic Type 1 Renal Tubular Acidosis due to Sjögren's Syndrome

Although renal calcium crystal deposits (nephrocalcinosis) may occur in acute phosphate poisoning as well as type 1 renal tubular acidosis (RTA), hyperphosphatemic hypocalcemia is common in the former while normocalcemic hypokalemia is typical in the latter. Here, as a unique coexistence of these two seperated clinical entities, we report a 30-yr-old woman presenting with carpal spasm related t...

Severe lactic acidosis and acute renal failure following ingestion of metformin and kerosene oil: a case report

INTRODUCTION Kerosene is a freely accessible hydrocarbon used in Sri Lankan (and other Asian) households for cooking and for lighting lamps. Kerosene poisoning is rarely reported among adults and its toxicological effects are not well known. Metformin is a commonly used oral hypoglycemic drug and its overdose leads primarily to lactic acidosis. Combined poisoning of metformin and kerosene and t...

Case report pH 6.68—surviving severe metformin intoxication*

Metformin, a widely used anti-diabetic agent of the biguanide family, although generally safe, holds the risk of developing a potentially lethal acidosis. The association between lactic acidosis and metformin is well-established but rarely seen in patients taking this medication. Its elimination relies solely on kidneys’ excretion, so its accumulation is feasible in just two circumstances: rena...

Lactic acidosis due to metformin therapy in a low risk patient.

A 55 year old diabetic women treated with chlorpropamide and metformin for three years presented with acute oliguric renal failure and lactic acidosis from which she died. The plasma metformin level was very high suggesting that the lactic acidosis was caused by the drug. There were no contraindications to metformin therapy and renal function was normal three months previously. This case demons...

Extreme lactic acidosis type B associated with metformin treatment

The elimination of metformin is exclusively through the kidneys and elevated plasma concentrations can cause lactic acidosis. We report a case of severe lactic acidosis (pH 6.60) occuring with ostensibly normal therapeutic doses of metformin in the setting of acute renal failure. Continuous veno-venous haemodiafiltration decreased plasma metformin concentrations from 266 lmol/L at presentation ...
